How to Register

To register for the O&G 2015 Live Surgery Workshop & Refresher Course, please complete one of the registration forms below and return it to the congress organisers together with your payment.

Pre-registration via the website will be possible until 20 November 2015. After this date only on-site registration will be accepted. It is essential that the instructions for payment of all fees involved are strictly followed. Every participant, including invited participants, must register.

All transactions are conducted in terms of Turners Conferences standard conditions of trading. They are published on the Turners Conferences website & available on application.

Payment

Registration fees are quoted in South African Rands (ZAR), and include 14% VAT and should be paid according to the procedure described on the registration form. Visa, MasterCard American Express and Diners credit cards or bank transfers are acceptable forms of payment.

When processing your payment by bank draft please allow sufficient funds to cover bank charges otherwise these will reflect as a short payment and show as a balance on your account.

What does the Registration Fee Cover?

Registration fee includes:
  1. Attendance at all sessions, light lunch, tea and coffee breaks, bag and entry to the Exhibition.

What is not included in the Registration Fee?

  1. Special catering requirements
  2. Attendance to the CTG Workshops.
  3. Any other items not mentioned.

Cancellation of Registration and Refund Policy:

*Please notify the Secretariat in writing of any cancellations, all approved refunds will be issued after the course. An administration fee of 10% will be charged for all cancellations received on or before 23 September 2015. Please note that no refunds can be made for cancellations made after 23 September 2015.
